Please print out the Client Intake Form below, complete it, initial, sign, date, and bring this form with you to your initial visit in-person or on Zoom Healthcare to download during your first virtual visit.  If you will be doing virtual, I will set you up on Zoom Healthcare after your Free Phone Consultation and call you back for the Meeting ID number and the Passcode.  I look forward to serving you!

Service Rates:

As a trauma expert, Elizabeth charges $200 per 50 minute virtual session. Elizabeth is an in-network provider for Blue Cross & Blue Shield and Medicare.
